Provide your vaccination record to UHS as soon as possible

The below message was sent to all SMPH employees on the morning of Wednesday, June 23, 2021. 

Dear colleagues,

As the university prepares to resume more in-person operations, accurate information on our campus community’s vaccination rate is vital.  Currently, we have a lower than expected percentage of SMPH employees with vaccination records on file at University Health Services.  If you have not done so, please upload your COVID-19 vaccination record if you received your vaccine off campus by following the directions here.

SMPH employees vaccinated anywhere other than UHS — such as through UW Health Employee Health Services, a pharmacy, your health care provider, or the Alliant Energy Center — need to either upload their record to UHS directly or give UHS permission to obtain their record from the Wisconsin Immunization Registry.  The process is fast and easy.  Please give permission or upload your vaccine record even if you currently work remotely.

Accurate information about our vaccination rate is important for preparing well-informed workplace health and safety plans.  This information is used for statistical and planning purposes only.  UHS will protect the privacy of your information.

Getting vaccinated is the most effective means of protection from COVID-19.  Vaccination helps safeguard the health of our school community and those we serve, including vulnerable colleagues and patients.  Thank you for uploading your records.
