Parking Information for Health Sciences Employees
First- and second-shift health sciences employees who wish to obtain a parking permit from UW-Madison Transportation Services must first fill out the parking application request form.
This is not the application for the parking permit itself. It is one-time process used to determine your eligibility for parking.
