Applications Sought for UW Health Community Service Awards

UW Health is accepting applications for the FY12 Faculty and Staff Community Service Award.
The award is designed to recognize three UW Health faculty and staff members who demonstrate exceptional commitment and leadership in a volunteer capacity to a community organization invested in promoting a community health priority.
The award will provide three recipients (one from UWMF, one from UWSMPH and one from UWHC) an award of $500. These funds will be donated in honor of the award recipient to the community organization where the award recipient volunteered.
Award applicants must have volunteered for a minimum of 50 hours on personal time (unpaid) within the past year (July 1, 2010, through June 30, 2011) for an eligible charitable health-related organization.
Applicants must be employed by UW Health at least half-time (0.5 FTE) or greater for at least two years and be an employee in good standing.
Applications will be accepted from Friday, July 1, through Friday, August 3. The award decision will be made by a selection committee, and applicants will be notified whether or not they were selected by Wednesday, August 31. The UW Health leadership team will present the award to the recipient and community organization in a special recognition ceremony to be held in fall 2011.
